La Jolla Eldercare Planning Council - Martha Abraham

ELDERCARE SAFETY AND QUALITY CARE EXPERT
Welcome to my service and my caring space. My name is Martha Abraham I am popularly known as the eldercare safety and quality care expert. I am a registered nurse with graduate education in healthcare quality improvement and patient safety.
I serve as the area director of La Jolla Eldercare Planning Council. In this role, my responsibility is to ensure that our council offers a one-stop service solution and care planning education hub to those who are dealing with the challenges that face the elderly.
I am responsible for the strategic partnership development between La Jolla Elder Care Planning Council and the business community, and faith-based organizations, social groups, and retirement communities. I also work closely with executives and professional families who do not want the stress of family caregiving nor the disruption of their work-life balance but want to see their aging loved ones THRIVE wherever they choose to call “HOME”.
I peel away the stress from family caregiving by providing compassionate family caregiving guidance, care advice, care navigation, complex care management, resources, and options for safe and graceful aging.
I come from a family of professionals and entrepreneurs and have seen firsthand the impact of family caregiving on personal health, personal relationships, finances, and work-life harmony. After one decade as a clinical nurse working in various care settings, I saw several gaps between well-being, safety, vitality & independence, especially for older adults whose loved ones are far away. I have combined my knowledge of nursing, my education in process improvement, and my passion for vitality and independence into a customized service for those families who want to avoid role reversal and enjoy work-life harmony while their aging loved ones thrive wherever they choose to call home on their terms regardless of any life-limiting conditions.
My clients are:
- Caregiving families seeking advice on the appropriate level of care and the appropriate setting for care.
- Busy executives who need caregiving guidance for their aging loved ones.
- Long-distance family caregivers who need help to navigate the elder care and retirement living industry.
- Assisted living facilities and retirement communities need navigators to help their residents age in place for as long as possible.
- Active retirees who want to remain as well and as independent as possible.
- Attorneys and guardians seeking help for their clients and wards.
What To Expect:
If the time has come for you to decide where and how your loved one will receive dignified and life-enhancing care, I am here to be of assistance to you. My phone is answered 24 hours a day, 7 days a week all year long. If you get my voicemail, your call will be returned within two business hours. Once we connect, you tell me your situation and we will explore possibilities and create a service plan that will take your stress away.
